Every one of these teams is going to the playoffs. One of them has already clinched the division, while the other two are obviously getting in.
Still, just because they are having strong seasons doesn't mean that they don't need to improve. This is what these teams need to do if they have any hopes on hoisting the Vince Lombardi Trophy in Indianapolis.

The 49ers have gone their last four games without scoring a touchdown in the first half. Ironically, they are 3-1 in that stretch and scored over 20 points in all three of those wins. With that defense, that number seems okay, but it will hurt this team down the road.
If everything goes according to plan, the 49ers will face the New Orleans Saints in their first playoff game. Of course, a lot can change, but that's looking like a probable showdown.
Now, that game would be in San Francisco, but the 49ers need to be more opportunistic. They move the ball well, but must take advantage of every chance they get. That applies to a potential game with the Saints and certainly applies when they play the Packers.
The 49ers have an excellent defense, but their offense needs to do a better job of putting points on the board early.
New England Patriots: Getting off of the field on defense
What's cost the Patriots in the playoffs is their inability to make key stops at big times. You simply can't go take on a playoff-caliber team and hope to score 30 or more points. It just doesn't happen often enough.
The Patriots' championship teams were defined by their defense. Tom Brady never put up massive numbers in those seasons.
It's not a bad thing that he does now, but it can't be the focus of the team. Bill Belichick and company need to find defensive schemes that get the opposing offense off of the field. If they do, they are the best team in the AFC. If not, they're going to lose their first playoff game again.
Pittsburgh Steelers: Running the ball/offensive line
While we're on the subject of AFC teams, let's go to the Steelers. Defensively, this team is solid. That's been the case in Pittsburgh since the 1970s, and it's certainly not stopping now.
What they need to do is find a way to run the ball more effectively. That just hasn't happened this year. On cold, windy, rainy days, deep balls to Antonio Brown and Mike Wallace just aren't reliable enough options.
They need to develop a better running attack, which starts up front. Running the ball more often will keep Ben Roethlisberger on his feet, which they also need to do. Without that, the low-percentage big plays aren't even possible.
