McIntosh joined France's Leon Marchand as the only swimmers to win two individual gold medals in Paris.
The 17-year-old was patient through the first 100 meters and then took control in front of Zhang and Smith.
McIntosh's pace at the end was so fast that she set the Olympic record previously held by Zhang's time in Tokyo. Zhang was comfortably in second place through 150 meters, but Smith produced an incredible final 50 meters.
Smith stayed underwater longer than anyone after the final turn and used that to her advantage to pass Zhang.
Smith earned her second consecutive silver in the 200-meter butterfly. She now owns four Olympic silver medals in her career. Zhang, who won the gold in Tokyo, took her second Olympic medal in the event with her bronze-medal finish.
