Chelsea Joy as Team Collects the Trophy

When the smoke clears, there is only one team that fully deserved to win the title this term.
Chelsea have epitomised the consistency a side needs to prevail in Europe's toughest league, and Jose Mourinho has once again stamped his card in west London.
It has taken The Special One just 24 months to transform Chelsea back into the best team in England, and the quest will be to now add European trophies to the mix.
The Blues swept past Sunderland 3-1 at the Stamford Bridge with two goals from Loic Remy and one by Diego Costa—capping a fantastic first season for the Brazilian-born Spain international striker.
The game also marked the end for Didier Drogba—playing his last game for Chelsea—as announced by their official Twitter account:

Drogba was made skipper for the day and was carried from the pitch by his teammates after being substituted in the first half, to delightful cheers from the Chelsea support.
Mourinho gave praise to his loyal striker, speaking of his influence on the team's most recent success, as quoted by ESPN's Miguel Delaney:

Chelsea's success this season has been built on an old warhorse, as well as dynamic attacking talent. Terry has played every minute of the EPL season, giving Chelsea the leadership they have missed in other seasons, when the defender's form has waned.
The former England skipper has provided five goals in the EPL this season, per Squawka, with the Blues losing only three games during the league campaign. Mourinho's success has been built around his defensive unit, and Terry has rolled back the years.
Chelsea will go again in 2016 and will be favourites to retain their title next season. Mourinho has the Midas Touch in England, and a sustained period of dominance could now happen.
Newcastle Save Themselves at the Death

The biggest team from the northeast almost found themselves in the Championship for next season, but Newcastle earned the win they needed to stay up.
The Geordies scored a 2-0 win at home against West Ham—who promptly parted company with Sam Allardyce at the end of the game, per Phil McNulty of BBC Sport.
John Carver's tenure at St James' Park has been a disastrous experiment by owner Mike Ashley, who has gambled with the affections of his club's supporters for too long.
Former legend and manager Kevin Keegan has spoke out against Ashley's ownership and said to BBC5 Live's Sportsweek he hopes the billionaire moves on, per BBC Sport:
"It is not difficult to work out unless you come from another planet, and I think Mike Ashley runs that club from another planet. Fans are being treated with such disrespect. Fans don't want to go to games; they want to protest."

On this occasion, Newcastle have got away with it. But the structure of the club means it will always struggle to compete until investing correctly in the squad and having an able manager.
Carver is a loyal servant to Newcastle, but he is simply not an EPL-standard manager. He has not shown the leadership needed during his tenure and will surely be shown the door.
However, if he continues, it will only further strengthen the idea Ashley does not have the team's best interests at heart—and supporters will have their right to turn their backs on their beloved club.
