Fans were left stunned by the Tar Heels' collapse after opening the tournament with a dominant first half.
VCU, a popular first-round upset pick heading into March Madness, was unable to find any answers for UNC's first-half defense. The Tar Heels took advantage to climb out to a 19-point midgame lead.
The Rams launched a furious comeback in the final five minutes of regulation, going on an 11-2 run to cut the deficit to two points with three minutes remaining.
UNC's Henri Veesaar answered at that point with a dunk to help keep the Tar Heels in the lead.
A late turnover gave VCU the ball with the score tied and 2.9 seconds left on the clock. UNC got the steal, but missed a potential buzzer-beating game-winner.
Both teams struggled to score early in overtime before Terrence Hill Jr. sank a three-pointer with 15 seconds left to put VCU up by two points.
Veesaar had the chance to tie the game on late free throws, but was unable to convert.
Hill finished the win with a game-high 34 points while coming off the bench for VCU. Veesaar led North Carolina, which was missing Caleb Wilson to a hand injury, with 26 points.
VCU will now advance to play either No. 3 Illinois or No. 14 Penn in the second round.
The Rams will learn the identity of their next opponent after the Quakers and the Fighting Illini close out their own first-round matchup Thursday night.
