Ramos previously dislocated his shoulder back in September against Shakhtar Donetsk and was subsequently absent for several weeks.
He would be a big loss for Rafael Benitez's side against Barcelona, which is why Real appear willing to nurse Ramos through one more match before allowing him to go under the knife. The veteran's leadership is vital to keeping the Real defence organised.
In Pepe and Raphael Varane, Los Blancos have the quality to cover for Ramos. However, given how crucial the Clasico could be in determining the La Liga title race, Benitez will want his first-choice pairing available.
