Both players battled hard in the first set, as Berrettini jumped out to a 3-0 lead before Arnaldi won three straight games.
Arnaldi eventually held a 6-5 advantage before taking the opening set after Berrettini saved the first set point.
Berrettini was eventually forced to take a medical timeout while trailing 2-1 in the second set.
He ended up playing four more games, but his movement became limited before he retired.
Arnaldi gave a shoutout to Berrettini following his win.
"We have both played a lot, it's normal not to be at our best," Arnaldi said, via Max Mathews of The Athletic. "It's not nice to end a tournament like this. He did such an amazing tournament. I'm sorry for him and I hope he recovers soon."
"I'm tired, that's for sure," he added. "But I train to play these kind of tournaments, and I'll try to give it everything I've got."
Arnaldi will now face Flavio Cobolliย in the French Open semifinals for an opportunity to clinch his first Grand Slam final appearance of his career.
